Hairline lowering with FUE is a hair transplant procedure in which follicles are moved from a donor area, usually at the back or sides of the scalp, and implanted in front of the existing hairline. This can create the appearance of a lower or reshaped hairline. It does not surgically shorten the forehead or advance the scalp.
For suitable patients, FUE hairline lowering may alter the shape and apparent position of the hairline. The degree of change that can be achieved depends on donor-hair availability, hair characteristics, existing or future hair loss and the planned design. Individual results vary.
At Dr Ducu Clinics, hairline lowering is performed using follicular unit extraction (FUE). Individual follicular units are taken from a suitable donor area and implanted into a planned recipient area at the frontal hairline. The number and placement of grafts are tailored to the patient’s donor supply, hair characteristics and agreed treatment plan.

1. Consultation
The first step is a consultation with the clinician who will assess you and plan the procedure. The assessment should include your medical history, pattern and stability of hair loss, scalp and donor-area examination, hair characteristics, expectations, treatment alternatives, limitations and potential risks.
2. FUE hairline lowering
Before having the procedure, we will administer local anaesthesia to ensure comfort throughout the process. Using the Follicular Unit Extraction (FUE) method, the surgeon will harvest individual hair follicles from the donor area, located at the back or sides of the scalp. This is done using a specialised surgical tool known as a micro punch.
A small punch is used to extract follicular units from the donor area. Punch size and technique vary according to the patient and clinician. FUE avoids a linear donor scar but still creates multiple small wounds and can cause visible scarring, altered sensation, bleeding, infection, over-harvesting or uneven donor density.
These follicles are then carefully transplanted to the frontal hairline to lower and reshape it, following the pre-determined design established during the consultation.
The grafts are placed according to the agreed hairline design. The aim is a natural-looking result, but graft survival, density, direction, scarring and cosmetic outcome vary between patients and cannot be guaranteed.
3. Aftercare & recovery
Following the surgery, you will receive detailed post-operative instructions. These may include guidelines for washing the scalp, avoiding strenuous activities, and taking prescribed medications to aid in healing and prevent infection. You will need to keep the treated area clean and protected, and avoid certain hair styling products or activities that could disrupt the healing process. We will also schedule regular follow-up appointments to monitor progress and address any concerns during the recovery period.
4. Results
In the months following the procedure, you will experience gradual hair growth as the transplanted follicles take root and begin to produce new hair. While initial results may be visible within a few months, full results typically become apparent after approximately 9-12 months, once the hair has fully grown and matured.
Growth is gradual. Early shedding of transplanted hairs is common, and visible regrowth usually develops over several months. Final density is commonly assessed at around 9-12 months or later. Results vary, not every graft survives, and further treatment may be required. Existing non-transplanted hair may continue to thin or recede over time.

Hairline Advancement - Requires about 1-2 weeks of recovery, with potential swelling and bruising around the forehead.
FUE Hair Transplant - FUE usually has a shorter initial recovery than open hairline-advancement surgery, but recovery varies. Swelling, redness, crusting, tenderness, itching, altered sensation and visible donor or recipient sites can persist beyond a few days. Patients should follow individual aftercare instructions.
Suitability:
Hairline Advancement - Best for individuals with a naturally high hairline and no significant hair loss, with good scalp elasticity.
FUE Hair Transplant - FUE may be suitable for selected patients with a high, receding or thinned frontal hairline who have adequate donor hair and a stable or appropriately managed pattern of hair loss. It redistributes existing follicles and does not stop the underlying process of future hair loss.

The results of hairline lowering are long-lasting and can be enjoyed for many years. Once the transplanted hair follicles have fully taken root and begun to grow, typically within several months after the procedure, the new hairline is considered stable and should remain in place. While natural hair thinning may continue to occur over time due to factors such as aging or genetic predisposition, the transplanted hair follicles are typically resistant to further hair loss.
Transplanted follicles that successfully establish may provide long-lasting growth. However, graft survival varies and the surrounding non-transplanted hair may continue to thin or recede. The long-term appearance is not guaranteed and maintenance treatment or further transplantation may be considered.
Hairline-lowering surgery using the FUE method typically lasts between 4 to 6 hours, depending on various factors such as the extent of the hairline-lowering required, the number of follicles to be transplanted, and the individual patient’s anatomy.
Washing instructions vary according to the technique used and your clinician’s protocol. You will be given individual written aftercare explaining when and how to wash the donor and recipient areas without disturbing the grafts.
After hairline lowering surgery you should avoid strenuous exercise, direct sunlight exposure, swimming, smoking, and traumatic handling of the scalp. Tight-fitting hats, headbands, or other accessories can put pressure on the surgical area and interfere with proper healing. You should opt for loose-fitting headwear. Also, sleeping on the stomach or sides can put pressure on the surgical area and potentially displace the transplanted follicles. You should try to sleep on your back with your head elevated to promote optimal healing.
You can expect to take about 1 to 2 weeks off from work to allow for initial recovery and to minimise physical exertion. However, for jobs that involve strenuous physical activity or prolonged periods of standing, additional time off may be necessary.
Local anaesthetic is used to reduce discomfort during the procedure. Patients may still feel injections, pressure or pulling, and discomfort, tightness, tenderness or altered sensation can occur during recovery. Experience varies between patients.
In some cases, another procedure may be considered, but combining treatments can increase procedure time, recovery requirements and risk. Suitability must be assessed individually, and there should be no pressure to undergo additional treatment.
As with any hair-transplant procedure, FUE hairline lowering carries risks and limitations. Common temporary effects include swelling, redness, crusting, tenderness, itching and numbness. Other complications can include bleeding, infection, folliculitis, visible donor or recipient scarring, prolonged altered sensation, shock loss of existing hair, poor or uneven graft growth, asymmetry, an unnatural hairline, over-harvesting of the donor area and dissatisfaction with the cosmetic result. Not all transplanted grafts will necessarily survive. Existing hair loss may continue, which can change the appearance of the result and may require medical treatment or further surgery. Individual results vary and no specific density, hairline position or cosmetic outcome can be guaranteed.
